You have the right to request a copy of your mental health records.

Cielo Azul Counseling, PLLC is required to retain client records for seven (7) years, or five (5) years after a former minor client reaches the age of majority whichever is greater, in accordance with Texas record-retention requirements.

To request your records from Cielo Azul Counseling PLLC, please:

  1. Submit a written request via your client portal or by email.

  2. Include your full name, date of birth, and the specific records you are requesting.

  3. Allow up to 15 business days for processing.
